Cheyenne Health and Wellness Center (CHWC)
strives to provide quality health care services to all those in
need, with integrity and respect, in a caring environment.
CHWC accepts all patients regardless of their ability
to pay for services. To ensure that income or lack of insurance
is not a barrier to care, low income patients who are not covered
by public or private insurance are charged on a sliding fee scale.
Patient Forms
To save time during the intake process, patients
may print and fill out patient information forms prior to thier
appointment.
Sliding Fee Schedule
CHWC's sliding fee schedule is based on family
household income. When applying for the sliding fee discount, all
household members must provide income documentation such as:
- Pay stubs for the last 30 days
- Employer statement of income
- Income tax return for the prior year
- Social security income statement
- Unemployment services statement
- Alimony
- Child support
If you are unable to provide income documentation at your first appointment, then a one day certification for the sliding fee will be allowed. Documentation will be required before your next appointment, or you will be billed the full fee for service.
CHWC Accepts Medicare, Equality Care,
Kid Care CHIP, and Private Insurance
Cheyenne Health and Wellness Center will
be happy to bill your health insurance for you as a courtesy. In
order for us to do so, you must provide a copy of your insurance
card. If this information is not provided, your insurance will not
be billed. Remember, this is only a courtesy. Cheyenne Health and
Wellness Center accepts:
- Blue Cross Blue Shield
- Medicare
- Equality Care (Medicaid)
- Kid Care CHIP (Blue Cross/Blue Shield)
- Other private insurance
Patients are responsible for pre-authorization
request, and should contact thier insurance company prior to thier
appointment.
Incomplete information may delay access to
beneficial programs available to you. Remember that you, the patient,
are ultimately responsible for payment of the services rendered.
